What to Bring

  • For married filing joint, both spouses must be present
  • All Forms W-2 and 1099
  • Information for other income
  • Information for all deductions/credits
  • A copy of last year’s tax return
  • Proof of account for direct deposit of refund
  • Social Security cards or Individual Taxpayer Identification notices/cards for you, your spouse, and/or dependents
  • Proof of identification for yourself and/or your spouse
  • Total paid to day care provider and their tax ID number
  • Birth dates for you, spouse and or dependents on the return
  • Proof of foreign status if applying for ITIN
  • Forms 1095-A, B or C (Affordable Health Care Statements)

Weather Policy

Our policy is to close our tax sites, if there is a Level 2 Snow Emergency. If you’re planning to file your taxes or scheduled to volunteer and you’re unsure whether you should head to your tax site, either check with your local radio or TV station to find out what the current level is or contact 2-1-1.Listed below are the definitions of each level (as defined by the State of Ohio).

  • LEVEL 1: Roadways are hazardous. Motorists are urged to drive very cautiously.
  • LEVEL 2: Roadways are hazardous with blowing and drifting snow. Roads can be very icy. Only those who feel it is necessary to drive should be out on the roadways. People should contact their employer to see if they should report to work. Obviously, if you are driving, use extreme caution.
  • LEVEL 3: All roadways are closed to non-emergency personnel. No one should be driving during this condition unless it is absolutely necessary or a personal emergency exists. All employees should contact their employer to see if they report for work. Anyone traveling on the roads may subject themselves to arrest.
